Key Details:
- Pay: $41- $45 per hour based on experience
- Bilingual Pay Policy: Eligible for a $1.25/hour increase for Spanish proficiency.
- Additional Perk: You will be paid time and a half from 6-8 PM.
- Schedule: Full-time; Monday – Friday; 35-40hr/week; currently hiring for swing shift with an 8PM end time with the option of 5-8s, 4-10s, or 3-12s.

What You’ll Do: As an RN Case Manager, you’ll support the development, implementation, and evaluation of individualized care plans for patients with complex medical and social needs. You’ll collaborate closely with patients, providers, and community partners to coordinate care and support better health outcomes.
RN Case Management Responsibilities:
- Partner with providers to support patient engagement and education.
- Conduct triage and provide nursing advice per protocols and standing orders.
- Complete patient appointments in alignment with RN standards of practice.
- Lead case management and care coordination meetings with patients.
- Support patients in setting health goals and navigating resources.
- Review and manage medication lists and refills per standing orders.
- Coordinate with grant teams and community partners as needed.
As a Wallace Team Member:
- Build strong relationships with patients, staff, and community partners.
- Uphold patient and organizational confidentiality at all times.
- Contribute to compliance, quality assurance, and quality improvement efforts.
- Participate in team meetings and trainings.
- Maintain a patient-centered, equity-focused approach to care.
What We’re Looking For:
You are a compassionate, adaptable RN who brings a collaborative spirit and a commitment to health equity. You enjoy patient education, complex case management, and partnering across teams to provide exceptional care.
Minimum Qualifications:
- Associate degree or higher in Nursing from an accredited program
- Current RN license in the state of Oregon
- Bilingual in Spanish (or other languages); multilingual candidates strongly encouraged to apply
- At least 1 year of RN experience in a clinical setting, or 2 years as a Medical Assistant
- Current CPR certification
- Familiarity with healthcare regulations and protocols
· Preferred:
- Experience working in a community health setting
- Knowledge of chronic disease registries and data-driven care coordination
