Key Details:
- Pay: $42–$48/hour depending on experience
- Bilingual Pay Policy: Eligible for a $1.25/hour increase for Spanish proficiency.
- Evening Bonus: Time and a half pay from 6–8 PM
- Schedule: Monday–Friday, full time, swing shift (8pm end time) with flexible scheduling options

What You’ll Do: As an RN Case Manager at Wallace, you’ll work with high risk, complex patients to improve outcomes and build trust. You’ll lead case management efforts, coordinate with internal and external partners, and help patients reach their health goals.
- Partner with providers and support patients in care planning
- Triage and provide nursing advice per protocols
- Lead case management sessions and set care goals with patients
- Coordinate resources and navigate referrals
- Review medications and manage refills
- Collaborate with grant-funded programs and community partners
What You Bring:
- Oregon RN license and current CPR certification
- Associate degree or higher in Nursing
- 1+ year of RN experience in a clinical setting (or 2+ years as an MA)
- Bilingual in Spanish strongly preferred (other languages welcomed!)
- A heart for mission-driven care and community impact
